If you believe you have completed this requirement through one of the options listed above, please click on the link for your specific graduation year below, and fill out and submit the form. Please DO NOT submit this form if you have not yet completed any of the community service graduation requirement options. Although the final deadline is the end of 12th grade, we strongly advise students to complete this requirement before the end of 11th grade.

Once a form has been submitted, it will be reviewed by Serrano Counseling Office staff. If it is determined that you have met the requirement, your transcript will be updated to show that you have earned the one credit for Community Service. However, if you have not met the requirement, you will receive an email explaining why, and what is still needed. You may contact the counseling office with any questions, at any time.

Please note that the graduating classes of 2025, 2026, and 2027 are EXEMPT from this graduation requirement, and do NOT need to complete a form!
