Student Documents
In order to conserve resources and ensure easier access for students and families, we will no longer be providing student packets at the beginning of the school year with the following forms and documents. All forms and documents will now be available at any time throughout the year through the links listed below. Hard copies will be provided upon request.
All Grade Levels:
- Honors/High Honors Graduate Requirements and Graduation Student Speaker Selection Process Students in all grade levels are strongly encouraged to review the requirements well before their senior year.
- California Colleges Opt-Out Form This is an OPTIONAL opt-out form and is NOT required. We strongly advise students and parents against filling out this form, as Serrano counseling staff and teachers use the free CaliforniaColleges.edu website with students throughout their four years of high school. Submitting an opt-out form will PREVENT your student from accessing the free resources and tools available on this site, including career discovery and college and financial aid application assistance. Please contact our office with any questions!
- McKinney-Vento Questionnaire Per California Assembly Bill 27, all families are required to complete this form annually. Completion of the form will help the district to determine if your child is eligible for McKinney-Vento Assistance services. The information provided on this form will be kept confidential and only shared with appropriate school district and site staff. Your child or children may have the right to:
- Immediate enrollment in the school they last attended (school of origin) or the local school where you are currently staying, even if you do not have all the documents normally required at the time of enrollment.
- Continue to attend their school of origin, if requested by you and it is in the best interest.
- Receive transportation to and from their school of origin, the same special programs and services, if needed, as provided to all other children, including free meals and Title I.
- Receive the full protections and services provided under all federal and state laws, as it relates to homeless children, youth, and their families.
11th and 12th Grade Only:
- CalGrant Opt-Out Form This is an OPTIONAL opt-out form and is NOT required. We strongly advise students and parents against filling out this form, as it will prevent your student from being considered for a grant from the state of California (i.e. free money to attend eligible college or career training schools). Please contact our office with any questions!
12th Grade Only:
- Selective Service Federal law requires men, ages 18 through 25, to register with the Selective Service System.
- Important Senior Counseling Dates - COMING SOON!
- FAFSA or CADAA (Financial Aid) Opt-Out Form This is an OPTIONAL opt-out form and is NOT required. We strongly advise all seniors who are considering attending any college or career training programs to complete a financial aid application, even if they do not think they will qualify for financial aid. Schools frequently rely on completed financial aid applications to qualify students for scholarships and grants that may not be based on family income, so it never hurts to just apply!
